CONDITIONS OF APPLICATION (must be read by owner before accepting booking)

Owner must be a resident of the Liverpool City Council Local Government Area.
Owner must have a valid pension card or healthcare card and be able to provide this at the time of appointment.
Owner must be able to pay the owner contribution on the day of appointment directly to the vet (before surgery). Extra charges are the responsibility of owner.
Owner’s animal must be microchipped or be done at time of desexing.
Owner must agree to Lifetime Register this animal within three (3) months of the surgery.
Owner must be made aware of and willing to accept the risks associated with general anaesthetic procedure.

By submitting this application I consent as the owner of the above animal, to the permanent surgical
desexing and microchipping of my pet by the chosen vet. I accept and understand that:

There is a risk associated with these procedures.
If my pet has not been vaccinated within the last 12 months, there is a risk that he/she may contract a
contagious disease during the hospital stay.
Liverpool City Council, RSPCA NSW, Department of Communities & Justice and the Vet Clinic will not be liable for
any illness or complication that arises from taking part in this program.
Program cannot be combined with any other Council offer and is available only to Liverpool residents.
Program is not available to rescue groups, organisations, breeders or trap neuter release (TNR) programs.
